Please complete the following informational form. We use this information to get to know you and your pet.  It helps us in determining the best services for him/her.  All of your information is kept confidential and used solely for the purposes of grooming and related administrative reasons. Please indicate when asked if you already have a scheduled appointment Yes or No. If no, you can use this space to express what days and times work best for you. You will then either receive a confirmation text (to the number you provided) with the first available date and time, or a phone call to determine what works best. You are not guaranteed this appointment until you receive our confirmation and respond with a “Y”.
